Indiana's workplace safety agency has ordered a record $453,000 in fines for violations at a central Indiana glass factory that weren't fixed after a worker's death nearly two years ago.
The Indiana Occupational Safety and Health Administration imposed the fines on Pilkington North America after inspectors found 29 violations at its Shelbyville plant.
Agency spokesman Bob Dittmer tells The Indianapolis Star and the Indianapolis Business Journal that the fines are its largest. The agency also fined Pilkington $150,000 last month for violations at the factory.
Dittmer says the Toledo, Ohio-based subsidiary of Japanese conglomerate NSG had numerous repeat violations from when a worker was fatally crushed in machinery in September 2010.
Pilkington spokeswoman Roberta Steedman says some of the issues have been corrected and others are being evaluated.
