A penalty from the Federal Government has IU Health's Starke Hospital implementing a new program. The penalty was issued because too many patients with certain conditions were being re-admitted too quickly.
Those conditions include pneumonia, heart attack, and congestive heart failure.
IU Starke's president says, because so many people with these conditions were readmitted, all hospitals are held accountable.
So, IU Starke Hospital will be starting the "transitional care" program to follow up with patients after they leave the hospital.
They say their goal is to keep patients out of the hospital.
