The Indiana State Department of Health published the school immunization requirements for the 2013-2014 school year and some new requirements have been added.
Students in grades kindergarten through 12th grade must have received two valid doses of the varicella vaccine, or have had a documented history of the chicken pox by a healthcare provider.
The department also recommends that kindergartners have two doses of Hepatitis A vaccine, and 11th and 12th graders receive a booster for the meningococcal conjugate vaccine.
Access to immunization records in Indiana is available through MyVaxIndiana’s website with the use of a personal identification number (PIN). Such PINs can be obtained through a patient’s healthcare providers, and then used to log into the website to see the records.
