LANSING, Mich. Prosecutors and clerks in 11 Michigan counties have failed to report or verify violations by concealed handgun holders.

MLive.com reports Thursday that a review of state concealed carry reports shows Kalamazoo and Livingston counties were among those that didn't properly report between July 2012 and June 2013.

The 11 failures are the most since 14 counties didn't report four years ago.

The 2012-2013 year had one of the largest increases in permits since reporting was mandated in 2001. There were about 118,000 concealed permits in Michigan in that period.

Kalamazoo County Clerk Timothy Snow says he wasn't aware the county wasn't reporting. He says he will follow up on it.

Livingston County Sheriff's Lt. Mike Kinaschuk says the county gun board likely "dropped the ball" during a personnel change.
